Insurance Information
We accept most insurance plans, HMO/PPO’s and Medicare. We understand the increasingly complex array of insurance requirements and claims procedures and will assist you by processing your claims. We do not however bill third parties for automobile accidents or personal injuries.
Please bring your co-payment, all insurance cards and an ID for your first appointment. If you change your insurance, it is your responsibility to notify our office and provide a copy of your new card. Our business staff will help explain billing policies to you if needed and work with you if you are unable to pay your account when due.
At the time of your visit, you should bring any referrals, X-rays, MRI’s or other studies. Please call if you are unsure what to bring. Co-pays, as mandated by your insurance, are expected at time of service.
Emergencies
Our physician or physician assistant is on call after regular office hours and weekends. Please call our main number (510) 704-7760.
Your call will be answered by our answering service, and the physician or the physician assistant will return your call as soon as possible. Please tell the answering service when you had surgery, what the problem is and your phone number.
Due to the unscheduled nature of emergencies, occasional delays in the appointment schedule do occur. We hope you understand that these delays are unavoidable, and that in an emergency you would be extended the same consideration.
Prescriptions
Prescription refills must be requested weekdays only during the hours of 9 a.m. and 4 p.m. Refill requests cannot be accommodated outside of regular office hours.
